Tutorial Description:  If the parent or guardian has active forms to sign, they will see the yellow banner as soon as they log in to myPueo. If there is no yellow banner, there are no active forms for the parent or guardian to sign. Perhaps another parent or guardian of the student already signed the form. Most School Forms require only one parent/guardian signature unless otherwise indicated.

1. If the form has not been completed or is incomplete, there will be a Review button next to it.

Click and follow the prompts to complete.

2. You can always view completed forms, even for prior years. Log into myPueo with your parent account.  Click on your name in the upper right corner of the screen.
3. Select Files & Forms.

• Click on Print to view any completed form.
