How to fill out Legal Guardianship Forms
Form 1: Legal Guardian Form
Form 2: Authorization and Consent of Parent(s) or Legal Guardian(s) (must be notarized)
Form 3: Medical Power of Attorney (must be notarized)
Options for Notarization:
- If the parent(s) is in Hawaii, the forms may be notarized at a bank by a notary public. $5 fee for each notarial service.
- If the parent(s) is not in Hawaii, the forms may be notarized at any U.S. Embassy or Consulate in their home country.$50 fee for each notarial service.
- If the parent(s) is living in one of the following regions under the Hague Apostille Convention (Hong Kong, Macao, Japan, Korea), the forms may be notarized by a local foreign notary in their home country with the attachment of an internationally recognized apostille.
