How to fill out Legal Guardianship Forms

Form 1: Legal Guardian Form

Form 2: Authorization and Consent of Parent(s) or Legal Guardian(s) (must be notarized)

Form 3: Medical Power of Attorney (must be notarized)

Options for Notarization:

  1. If the parent(s) is in Hawaii, the forms may be notarized at a bank by a notary public. $5 fee for each notarial service.
  2. If the parent(s) is not in Hawaii, the forms may be notarized at any U.S. Embassy or Consulate in their home country.$50 fee for each notarial service.
  3. If the parent(s) is living in one of the following regions under the Hague Apostille Convention (Hong Kong, Macao, Japan, Korea), the forms may be notarized by a local foreign notary in their home country with the attachment of an internationally recognized apostille.
